Designing Balance Exercise Programs for Older Adults. BY LANCE DALLEK, Ph. D. Fall incidence rates currently pose a serious health problem for older adults. Among those who are 6. Decreased balance is attributable to an age- related decline in multiple physiological systems that contributes to decreased muscle flexibility and strength, reduced central processing of sensory information, and slowed motor responses (American Geriatrics Society, British Geriatrics Society, and American Academy of Orthopedic Surgeon Panel on Falls Prevention, 2. In addition to an increased risk of falls, diminished balance and mobility may limit activities of daily living or participation in leisure- time activities. Accordingly, it is essential that balance exercises be incorporated into the physical activity programs of older adults. The following article uses case studies to outline the critical components of designing individualized balance exercise programs for clients that reduce their risk of falls. Sample balance exercises and training progressions (from simple to complex) will also be presented. Origin of Poor Balance and Falls An understanding and knowledge of the factors contributing to impairments in balance is fundamental to designing an effective balance- training program. Poor balance is multi- factorial in origin; in particular, cognitive impairment, visual disturbances and poor reaction time increase the likelihood of falls. Moreover, balance impairments and lower- extremity skeletal muscle weakness are also well- recognized independent contributors to falls. In particular, fallers have less muscular strength in the quadriceps and ankle dorsiflexors and plantarflexors compared to non- fallers (Orr et al., 2. It should be noted, however, that previous research exploring exercise training as a means for fall prevention in older adults has shown conflicting results (Mansfield et al., 2. While some studies have reported a reduced risk of falls after exercise training, other research has reported limited or no benefits. Even so, there are important lessons to be learned from these incompatible findings, and the complex etiology of poor balance highlights the need for a comprehensive intervention program. With that in mind, consider the following six critical issues that must be considered when developing an all- inclusive strategy for optimizing balance training and fall prevention among older clients. Multi- component training is superior to single- component balance training. The explanations generally given for lack of a favorable adaptation from balance exercises are 1) lack of specificity with training, and 2) performance of single- component compared to multi- component training. It has been reported that training programs including only single- task activities fail to place the client in an environmental condition similar to that experienced prior to and during a fall (Silsupadol et al., 2. Importantly, though balance training focused on improving functional tasks (e. A well- designed exercise program should feature concurrent performance of balance exercises and additional tasks. For example, in addition to performing heel- toe walking, the client may simultaneously be asked to complete a cognitive task, such as counting backward from 1. An additional form of dual- component training may involve combining a balance exercise with another form of physical activity. For instance, the client could be asked to balance on one leg while playing catch with a light medicine ball. In summary, multi- task balance training more closely replicates the activities of daily living in which a client’s balance performance is most likely to be challenged by a disturbance. Simulate loss of balance during training. Balance- training programs and fall- prevention interventions must include a focus on balance- recovery reactions (Mansfield et al., 2. Ultimately it is the capability—or lack thereof—to recover from a balance perturbation (loss of balance) that eventually determines whether or not a client falls. Balance disturbances can arise from collisions, slips and trips. Additionally, loss of balance can occur during voluntary movements, including bending, reaching and turning. Our body has a natural line of defense against balance disturbances: rapid limb movements. For example, reaching out to grab a supporting object or quickly stepping forward with a lower limb are compensatory mechanisms aimed at preventing a fall. Accordingly, it is logical to address the balance- recovery skill levels of clients because effective training programs will be those that replicate sudden and unpredictable balance disturbances. Importantly, to elicit the most favorable adaptations in a client’s balance- recovery reaction capacity, the exercises and activities should be designed in a way that does not permit the client to anticipate a balance perturbation. Couple resistance training with balance training. Despite the fact that poor balance is frequently associated with reduced muscular strength, the literature does not currently support resistance training alone as a successful strategy for enhancing balance performance and fall reduction. A systematic review of the efficacy of resistance training as an isolated intervention for uniformly improving balance revealed that this approach was successful in only one of five instances (Orr et al., 2. Therefore, it is crucial to remember that resistance training needs to also be coupled with balance training for positive modifications to be conferred on postural stability. Indeed, the integrated exercise- training approach has been found to be effective in the literature (de Bruin and Murer, 2. Correctly sequence balance exercises. Aerobic, resistance, flexibility and balance training are each critically important for the overall health, functional capacity and quality of life of older adults. However, to fulfill the minimum frequency requirements of each form of activity, clients will need to perform at least two (or more) activities on the same day, and most likely within the same exercise session (Nelson et al., 2. Research has reported that participation in either resistance or flexibility activities prior to balance exercise can negatively impact performance (Behm et al. For the senior client who may already face significant balance challenges, it would be inappropriate (and possibly harmful) to create additional perturbations due to improper activity sequencing. Balance training (when combined with resistance and flexibility activities) should be performed first or following aerobic activity. Create innovative balance exercises. Conventional balance- training programs include various sitting and standing activities, which, for the motivated client, has been shown to be effective over the long- term. Is it safe for seniors to exercise? According to the American Academy of Family Physicians, almost all elderly individuals can benefit from additional physical activity. HASfit Exercise for Elderly - Seniors Exercises for Elderly - Seniors. Why yes and –MusclePharm scientific nutrition example exercise programs for the elderly Few of us to focus and most. You asked for an update of OLR Report 2008-R-0021 which describes state programs for the elderly. The state ' s principal elderly services programs provide. Since accidental falls are a significant source of injury for many seniors, including balance exercises in your exercise regimen is essential. However, in less- motivated individuals the performance of repetitive, basic tasks can lead to poor adherence, less- effective training and ultimately cessation of training. Consequently, continuously designing novel and creative balance exercises for clients is essential. Recent research has reported that utilizing interactive video games may be an effective strategy to employ with designing balance activities for older adults (Nitz et al., 2. For example, the Wii Fit has various balance modules, including soccer, skiing and penguin, which can be performed at different skill levels, depending on the client’s functional capacity. Science has shown that progressively incorporating interactive video games into training can increase motivation and improve balance performance (Betker et al., 2. Change the availability of sensory cues. How can you design your personal exercise program? Use six steps for planning exercise types, intensity, frequency, and duration to reach your goal. Welcome to Exercise & Physical Activity: Your Everyday Guide from the National Institute on Aging! The National Institute on Aging (NIA) is part of the. Regular physical activity helps improve your overall health and fitness, and reduces your risk for many chronic diseases. Many falls occur during conditions that are unfamiliar to the client. Poor lighting or uneven surfaces impair the sensory cues typically available, therefore temporarily compromising the balance performance. These challenges can be introduced into the training program as a means to better prepare clients for circumstances where sensory cues are unavailable (Silsupadol et al., 2. For instance, the client might perform heel- toe walking while wearing sunglasses (inside), with eyes closed, or while slowly turning the head from side- to- side. Additionally, standing balance exercises can be completed while standing on a foam pad or balance disk in an effort to disturb the surface conditions. Regardless of which techniques are utilized, altering the sensory cues available to a client is an important consideration when preparing the overall balance- training program. F. I. T. T. Recommendations for Balance Training. The frequency, intensity, time and type (F. I. T. T.) approach to exercise program design can also generally be applied to balance- exercise programming (ACSM, 2. Although research has yet to identify the optimal frequency, intensity, duration and type of balance exercises, it has been recommended that balance training be performed for 1. Nelson et al., 2. Balance training should be integrated into the overall physical activity program according to the sequencing guidelines discussed earlier. Effectively Progressing Balance- training Exercises. A program designed to optimize balance training and fall reduction and prevention should incorporate each of the key points discussed earlier. Clients with no previous balance- training experience should initially perform basic sitting and standing exercises as a means to improve balance performance. As these initial exercises become easier, an increase in difficulty can be accomplished in numerous ways (ACSM, 2. Arm progressions: Vary the use and position of the arms to make a given balance exercise more difficult. Initially, the client may need to grasp or touch another object, such as a wall or back of a chair, to facilitate balance. Progressively, exercises can be performed with arms spread out and raised to shoulder height to assist with stability. Ultimately, clients can move arms in from sides to a folded position across the chest. Min Senior Workout - HASfit Exercise for Elderly - Seniors Exercises for Elderly - Seniors. A nursing home, convalescent home, skilled nursing facility (SNF), care home, rest home or intermediate care. It is a place of residence for people who require continual nursing care and have significant difficulty coping with the required activities of daily living. Nursing aides and skilled nurses are usually available 2. Residents include the elderly and younger adults with physical or mental disabilities. Residents in a skilled nursing facility may also receive physical, occupational, and other rehabilitative therapies following an accident or illness. The New Poor Law curbed the cost of poor relief, which had been spiralling throughout the previous decades, and led to the creation of workhouses for those who were unemployed. Most workers in the workhouse were set tasks such as breaking stones, bone crushing to produce fertilizer, or picking oakum using a large metal nail known as a spike. Although conditions in the workhouse were intended to be harsh, to act as a deterrence, in areas such as the provision of free medical care and education for children, inmates were advantaged over the general population. By the late 1. 84. London and the larger provincial towns housed only . Responsibility for administration of the Poor Law passed to the Local Government Board in 1. By the end of the century only about 2. The Local Government Act of 1. London few did so. Although the Act formally abolished the workhouse system in 1. Public Assistance Institutions, continued under the control of local county councils. A nursing home, convalescent home, skilled nursing facility (SNF), care home, rest home or intermediate care provides a type of residential. This program prompted many new nursing homes to be set up in the following years. Many of the buildings were converted into old folks' homes run by local authorities. In the 1. 98. 0s a significant shift from the public sector provision of elderly care to private sector homes occurred, with the proportion of private facilities increasing from just 1. This program prompted many new nursing homes to be set up in the following years, although private nursing homes were already being built from the 1. Great Depression and the Social Security Act of 1. Nursing home staff.
In the U. S., for instance, nursing homes must have at least one registered nurse available for at least 8 straight hours a day throughout the week, and at least a licensed practical nurse on duty 2. Depending on the size of the nursing home, the administration staff may be very small, consisting of only a handful or people, or it may have dozens of staff responsible for individual departments (i. In most countries, nursing home administrators are required to be licensed to run nursing facilities. This includes registered nurses, licensed practical nurses and nursing assistants. Support personnel. Custodians, maintenance staff, and groundskeepers, for example, keep the inside and outside of the building in clean, working order. Additional support personnel also include people who may have some contact with the patient in the nursing home, but it may not be daily or even regularly. For example, nursing homes may have an activities director who is responsible for planning and implementing holiday events, daily and weekly educational and social activities, coordinating special visitors and religious services. Larger facilities may have multiple staff members, such as chaplains or activity assistants, who take on some of those roles. Physical therapy staff may also be available, depending on the home. Nursing homes offer help with custodial care. Skilled care also includes services provided by specially trained professionals, such as physical, occupational, and respiratory therapists. Services can include: Room and board. Monitoring of medication. Personal care (including dressing, bathing, and toilet assistance)2. Social and recreational activities. Occupational Therapy. Occupational Therapists . For example, Occupational Therapists can take a leadership role in developing and implementing programs to educate clients on compensatory techniques for low vision, customized exercise programs, or strategies to prevent falls. Occupational therapy practitioners may also consult with other staff within the facility or in the community on a variety of topics related to increasing safe engagement in activities. Occupational therapy practitioners can provide a variety of services to short- and long- term residents of a SNF. Based on a client- centered evaluation, the occupational therapist, the client, caregivers, and/or significant others develop collaborative goals to identify strengths and deficits and address barriers that hinder occupational performance in multiple areas. The intervention plan is designed to promote a client. This can be for any number of reasons. Perhaps a person has motor skills that never fully developed or have stopped functioning for some reason. Perhaps an individual has undergone a surgery or medical procedure that requires some manner of physical restitution on a personal level. Nursing homes offer specialists that are well versed in the field of rebuilding muscle or helping one regain their confidence when it comes to doing something physical. This is one of the most common therapies that are done in these nursing homes. Most nursing facilities are equipped to deal with most general medical needs likely to emerge. Most of the staff will have ample training in how to deal with patients with specific needs. In fact, the staff that interact daily with the patients are normally registered nurses, who have spent years training for all contingent situations that they may encounter in a nursing home. These regulatory bodies are usually tasked with ensuring patient safety for the residents and improving the standard of care. Centers for Medicare and Medicaid Services ensures that every Medicare and Medicaid beneficiary receives seamless, high- quality health care, both within health care settings such as nursing homes, and among health care settings during care transitions. To ensure that nursing homes meet the necessary legal standards, the authorities conduct inspections of all nursing home facilities. This process plays a critical role in ensuring basic levels of quality and safety by monitoring nursing home compliance with the national legal requirements. Surveyors will conduct on- site surveys of certified nursing homes on average every 1. The authority might also undertake various initiatives to improve the effectiveness of the annual nursing home surveys, as well as to improve the investigations prompted by complaints from consumers or family members about nursing homes. Some have tried to create a more resident- centered environment, so that they should become more . Team nursing would ideally respond to the needs of both the patient and the staff. The team leader's function is to stimulate the team to learn and develop new skills. The team leader instructs the team members, supervises them, and provides assignments that offer them potential for growth. Team nursing is characterized by the following. The team leader should discuss the needs of the patients, establish goals, individualize the plan of care for each patient, instruct the team members, and follow up on all directions previously given to the team. The nursing care plan is a written guide that organizes information about a patient's health. It focuses on the actions that must be taken to address the patient's identified nursing diagnoses and meet the stated goals. It provides for continuity of care by a constantly changing nursing staff. The team leader starts the care plan as soon as the patient is admitted to the medical treatment facility. In response to changes in the patient's condition, and evaluation of goal achievement, the nursing care plan is updated and revised throughout the patient's hospital stay. With resident- oriented care, residents are able to make more choices and decisions about their lifestyle. Their families are more involved in the residents care, and employees have a greater degree of participation with the residents. Resident- oriented care combines the clinical models of care with a flexible social models. Every member of a facility's team, care for the residents, from administrators to the nurse aides. For example, all call lights are answered immediately. Whoever is close when the call is placed, responds - even if this is a member of the administration. Public and subsidized differ only in their ownership, all other aspects of funding, admission criteria, cost to the individuals are all regulated by the Quebec Ministry of Health and Social Services.
